When Barb Herrera looks back at her life just three years ago, she barely recognizes the woman she used to be. At over 400 pounds, bedridden, and relying on a walker just to reach the bathroom, Barb had nearly given up hope. Today, she’s 280 pounds lighter and walking up to eight miles every morning. Her secret? A combination of determination, small daily wins, and the life-changing impact of GLP-1 medications.
A Lifetime of Struggle
Barb’s relationship with weight began early. At just 10 years old, her doctor put her on amphetamines to manage her size. From that point forward, diets, pills, and quick fixes became a familiar cycle. She tried Fen-Phen in the ’90s and later underwent gastric bypass surgery in 2001. But nothing lasted.
It wasn’t until a physician told her she was on track for a nursing home that something clicked. That moment, combined with the introduction of GLP-1 medications, became the turning point. “He saved my life,” Barb says. “I had to choose to live.”
Starting from Zero
At her heaviest, basic self-care was impossible. Showers were exhausting, spaced weeks apart. She needed tools to clean herself and assistance from her kids. Her food came almost exclusively from Uber Eats, often in portions meant for a family. She also drank up to 10 cans of Diet Coke a day and used alcohol to cope with her depression and chronic pain.
Her first step wasn’t glamorous. “I just started lifting my arms and legs in bed,” she says. It took her three months to manage stairs, and eight months to reach the mailbox. Now, she walks seven to eight miles daily, hits the gym regularly, and proudly wears a shirt that says, “I lost 280 pounds and gained a life.”
The Power of GLP-1s
Barb credits Mounjaro with turning off what she calls the “food cacophony”, a nonstop mental loop of cravings, food planning, and shame. “The moment I took Mounjaro, everything got quiet,” she says. “I had room in my brain to live again.”
She also experienced remission from multiple health conditions: type 2 diabetes, liver disease, heart failure, GERD, and even a dramatic improvement in chronic kidney disease, an outcome her doctors didn’t think was possible.
A New Relationship With Food
Barb didn’t count calories or macros to lose weight. Instead, she started with what was manageable: Lean Cuisine, Healthy Choice meals, plain chicken with a touch of barbecue sauce, and yogurt with granola. “I don’t binge anymore,” she explains. “And if I eat out, I just leave what I don’t need. It’s better in the trash than in my body.”
Now in maintenance, she allows herself small treats, like half a peanut butter and jelly sandwich, and continues to listen to her body.
Finding Purpose in Her Pain

As a retired midwife and doula, Barb was no stranger to caring for others. Today, she uses that same compassion to support people with obesity who feel forgotten or judged. She runs a blog, healthatanycost.com, and a YouTube channel to share her story and practical tips, like using soup cans as beginner weights or how to navigate fitness with mobility issues.
“There’s such despair among people over 300 pounds,” she says. “I want to be a beacon of hope.”
Reclaiming Joy
Barb’s transformation has allowed her to show up for life fully, and most importantly, for her five grandchildren. She’s flown on planes without needing extra seats, jumped on a trampoline for the first time, and walked through Disney World without a wheelchair.
“I’m finally living a life,” she says through tears. “Not just existing in a room.”
A Message for the Critics
To those who say using GLP-1s is the “easy way out,” Barb has a message: “Come live in my world for a day. I worked my ass off to get here.”
Barb’s story is proof that these medications can be powerful tools, but they work best when paired with grit, support, and a willingness to do the emotional work.
